【摘要】 目的 观察重症监护室（intensive care unit，ICU）脓毒症(sepsis)患者的前白蛋白(prealbumin，PA)的变化，分析其与免疫功能、疾病严重程度以及死亡率的关系。方法 对收治ICU的15例Sepsis患者进行全肠外营养（total parenteral nutrition，TPN），观察Sepsis患者TPN前后PA、白蛋白、T淋巴细胞亚群、NK细胞、免疫球蛋白的变化，分析PA与白蛋白、T淋巴细胞亚群、NK细胞、免疫球蛋白以及预后的关系。结果 收治15例Sepsis患者中PA均明显下降；但T细胞亚群下降不明显，免疫球蛋白中补体C3、补体C4有下降，其余下降不明显。结论 血清PA的检测可能是预测Sepsis患者疾病严重程度以及死亡率的敏感性指标。

The change of prealbumin with sepsis and it’s clinical significance
【Abstract】 Objective To explore the change of prealbumin concentrationsduring sepsis in the intensive care unit of The First Affiliated Hospital of Medical College of Zhejiang University，and to analyse the relationship among prealbumin，illness degree，immune function and the mortality rate.Methods Total Parenteral Nutrition therapy were used on all the sepsis patients.To observe the change of prealbumin，T-lymphocyte，Nk cell，HLA-DR，immune globulin and the prognosis before the TPN and after the TPN separatly.And to analysie these data.Results A marked decrease in prealbumin was observed at all sepsis patients，most Immune function index fluctuated within normal range，but complement C3，C4 which were decreased.And through the TPN，a marked increase in prealbumin was observed，but other indexes showed no significant changes.Conclusion The results suggested that the prealbumin is the sensitivity index to predict illness degree and the mortality rate of sepsis patient.
