2型糖尿病伴发抑郁症病人自我管理行为的研究

 摘 要
 目的
 糖尿病患者存在较多的心理健康问题，其中抑郁及与抑郁相关的症状不但是构成２型糖尿病发生发展的主要危险因素，而且对２型糖尿病患者的心理、生理功能、并发症的发生，血糖调节及治疗等方面也会产生复杂而显著的影响。探讨糖尿病患者中抑郁症的发生率以及其相关因素。
 方法
 研究对象为由我院内分泌病房确诊为2型糖尿病患者100例。对入选的糖尿病患者进行问卷调查，询问调查内容患者一般资料，合并疾病和并发症情况等；HAMD抑郁量表。2型糖尿病患者抑郁症与相关因素的研究采用非条件logistic回归分析，所有统计用SPSS17.0软件包完成。
 结果
     抑郁症患者有45例，占调查人群的45％。其中轻度抑郁患者33例，占33.3％；中度抑郁患者10例，占10％；重度抑郁患者2例，占2％。抑郁症相关因素显示：病程、文化程度、性别是2型糖尿病合并抑郁症的独立危险因素
 结论
 糖尿病患者常存在焦虑、抑郁心理，且病程、文化程度、性别是2型糖尿病合并抑郁症的独立危险因素，所以加强糖尿病知识宣教，给予针对性心理护理是必要的。

 Title: Analysis of related factors for depression in type 2 diabetes
 ABSTRACT
 Object: There are many mental health problems with diabetes, while conventional drug treatment be necessary psychological intervention and antidepressant drug treatment of diabetes disease control efficacy is yes, it can reduce complications and wells adverse events and improve quality of life of diabetic patients, diabetic patients in the early to recover. This study was to explore the path to increase the self-efficacy by analyzing the correlations of self-efficacy and demographic data, knowledge on diabetes mellitus psychological state，social supports，disease control state as well as treatment measures in diabetes patients．
 Methods: 40 patients were recruited from hospital according to WHO type 2 diabetes mellitus diagnosis standard in 1 999．Those wim psychiatric disorders and malignant tumors were excluded in this study．The study was carried out by questionnaires，which included demographic data，diabetes related knowledge，self rating depressive scale，social supports rating scale and chronic disease self-efficacy.
 Results: Social supports and self-efficacy：unvariate and multi—variable logistic regression analysis showed that social supports including objective and supports contributed to the increased the scale of self-efficacy subjective.
 Conclusions：Therefore，increasing efforts on directly intervention，mental nursing，social supports and health education are necessary to improve the scale of self-efficacy, which could further contribute to the effective treatments，delayed and controlled complications and improved quality of life．
