人工血管表现内皮细胞种植研究的进展

摘要 材料植入机体诱导的血栓形成是人工血管临床应用最为主要的限制因素之一,在材料表现种植内皮细胞可能是目前解决这一问题最有效的方法,本文试就血管内皮细胞的促凝及抗凝功能、内皮细胞在材料表现生长分化的影响因素和人工血管内皮细胞种植的实验室研究与临床等方面的最新进展及其应用前景作为简要介绍。

1 引言
  血栓形成是人工血管尤其是微小血管临床应用的最为主要的限制因素之一,为了改善血管材料的血液相容性,人们曾先后进行了多方面的尝试,如新材料的合成、材料表现的生物改性等,遗憾的是目前的结果仍不能令人十分满意。血管内皮细胞不仅仅是在血管表现的一层物理屏障,更为重要的是其在血管伸缩的调节,凝血一纤溶系统平衡的维系以及免疫系统的调控等方面起着不可缺少的功能和作用,同时它还参与机体的生物代谢、能量运输和内分泌系统的调节。现在人们已起来越明确地认识到内皮细胞衬层在改善血管材料血液相容性中的重要性,在这方面的研究也日益引起人们的广泛关注并已取得了可喜的进展,本文试就近几年的有关进展加以综述。

作者点评:
 通过对上述研究工作的了解,可以看出,血管材料表面内皮细胞化能够明显改善移植后的血管通畅率,因而具有广阔的临床应用前景。但要使血管内皮细胞化人工血管真正进入临床广泛使用,还有几个问题迫切需要解决;一是如何缩短内皮细胞的种植时间从而减少污染的机会;二是要彻底了解影响内皮细胞在材料表面粘附、铺展、生长及功能分化等方面的各种因素,为日后材料的制备提供理论基础。更为理想的是研制这样一种血管材料,当其植入体内后能诱导邻近血管平滑肌细胞和内皮细胞向其表面迁移并进行铺展,从而形成一种完整的具有正常生理功能的内皮细胞层,这样将会使其临床应用更为简易。此外在体外培养条件下进行血管诱导再生的研究也应引起我们足够的重视。
