近些年来,微循环理论与实践研究进展是多方面的,但也存在某些争议问题,如微循环的概念及其血管网络范围界定,微循环调节和内皮细胞功能,疾病过程的微循环机制和微血管病等。本文就上述问题复习近期文献进行综述性探讨和评价。
微血管网络结构
微循环包括微动脉、毛细血管和微静脉。微动脉不断分支,越来越细,分为1、2、3、4级微动脉(即A1、A2、A3、A4)。按解剖名称分为喂养微动脉(feeding arteriole)另称喂养小动脉(small feeding artery)、弓形微动脉包括弓桥形微动脉(arcade bridge arteriole)和弓形微动脉(arcade arteriole)、横贯微动脉(transverse arteriole)或称终末微动脉(terminal arteriole)和同等水平的后微动脉(Metarteriole)及其延续的毛细血管前括约肌(precapillary sphincter)。微动脉壁成份有内皮细胞、基底膜、内弹力板、平滑肌细胞、外弹力板和外膜。各器官微血管构型、结构和功能各有独特标志,各级微血管管壁结构亦不相同。许多组织中微动脉含有1~3层平滑肌细胞,呈螺旋状环绕排列,到毛细血管起始端的平滑肌,称为毛细血管前括约肌。微动脉系统是调节血流的阻力血管,口径范围为5~100μm,包括多层平滑肌的较大微动脉和单层平滑肌的较小微动脉,和毛细血管前括约肌一起组成毛细血管前阻力。
