腕关节韧带是一个高度分化的复杂连接体系,不但具有限制过度活动、稳定腕关节的作用,而且还有传导应力,协调腕骨运动的功能。近年来,腕关节不稳定已逐渐被临床医生所认识,对腕关节韧带解剖和功能的研究也受到重视。以往的研究对腕关节韧带的起止、走行及命名有许多不一致的地方。现将近年来对腕关节韧带解剖及生物力学特性研究进展综述如下。
1 腕关节掌侧韧带
1.1 桡腕韧带和尺腕韧带
(1)桡舟韧带(radioscaphoid ligament),过去一些作者称其为桡侧副韧带[1],但它不是位于腕关节的侧方,而是偏向掌侧,关节屈伸运动轴由其背侧穿行,在解剖上它不是真正的侧副韧带。因此,目前大部分学者称其为桡舟韧带[2、3]。Mayfield报告桡舟韧带的断裂强度平均为70N[4]。(2)桡舟头韧带(radioscaphcapitate ligament),也有人称其为桡头韧带(radiocapitate ligament)。其断裂强度平均为170N[4]。(3)桡月韧带(radiolunate ligament),也有学者称其为长桡月韧带(long radiolunate ligament)[5],此韧带较为强韧,是稳定月骨的重要韧带之一,其断裂强度平均为210N[4]。Mayfield将桡月韧带和月三角韧带认为是1条韧带,并称之为桡三角韧带。而目前许多作者已证实此韧带实际上是2条韧带,因为有各自的起止点。(4)桡舟月韧带(radioscapholunate ligament),Mayfield称其为桡舟韧带 目录:
