中国协和医科大学 心血管研究所 阜外心血管病医院 * 博士生 传统支架生物瓣设计与天然心脏瓣膜相差很大,由于瓣架的存在造成应力的集中,易导致瓣叶撕裂,二尖瓣失功〔1,2〕。传统生物瓣替换需要在二尖瓣环处植入硬环,术中不保留二尖瓣瓣下结构的完整性,这些都可影响术后效果和左心室功能。为了避免支架生物瓣的缺点,使生物二尖瓣更加符合生理要求,加之近年来对二尖瓣装置生理功能认识的深入,无支架生物二尖瓣再度受到重视。现就这方面的一些进展作一综述。
无支架生物二尖瓣的种类
根据无支架生物二尖瓣材料的构成可分为以下几类:
1.同种瓣:包括同种二尖瓣、主动脉瓣、肺动脉瓣。早在60年代就已开始同种二尖瓣替换的研究,由于当时手术方法和组织保存等限制,结果不很满意〔3〕。最近Acar等〔4〕报告43例病人接受超低温保存同种二尖瓣替换手术,其中21例为部分瓣膜替换,22例为全瓣膜替换,术后效果良好。随访14个月,超声心动图检查20例无二尖瓣反流,13例轻度反流,5例中度反流;瓣口面积:部分瓣膜替换为2.5±0.4cm2,全瓣膜替换为2.7±0.4cm2,跨瓣压3±4mmHg(1mmHg=0.133kPa)。
